Create an 8-Project Proposal. Your proposal should incorporate the work you have completed in the previous 4 weeks (see attachments). It should answer the 5 W’s (who, what, when, where, and why) and H (how) regarding your project:
• Why is there a need for the project?
• What is the project, and what do you intend to accomplish with it?
• Who will participate in it, and who will be impacted by it (i.e., stakeholders)?
• Where will the project take place?
• When will it take place?
• How will you implement the project, and how will you evaluate if the intervention addresses the need?
The aim of the proposal is to secure the buy-in from your project stakeholders. The proposal should sell itself. The more comprehensive a plan you present in the proposal, the greater the odds of buy-in. You will want to use this at your kick-off meeting to gain consensus among all stakeholders and set the expectations and common objectives for your project.
Your final practicum proposal should contain all of the following elements:
• Table of contents
• Introduction
• How the project aligns to the organization’s vision and mission statement
• Background of project’s impetus
• Literature review/synthesis
• Gap analysis
• Project charter, scope, and objectives
• Implementation plan
• Define the best practices to be implemented.
• Define the task, actions, deliverables.
• Define the metrics that will determine success. How will you know that change is an improvement?
• Timeline and schedule
• Budget
• Cost-benefit analysis
• Identification of all resources needed (tangible and intangible)
• Risk management plan
• References page
• Appendices
Note: The expected length of the final practicum proposal document is 8 to 10 pages, not including the title page, table of contents, references page, and any appendices. You may incorporate some of the tools you completed into your narrative or as appendices to strengthen your proposal. All remaining work that you completed for your project should be included as appendices to the proposal.

Proposed project information attached. Quick overview—In the present care, there is a problem with the compliance of DVT/VTE protocols in the healthcare organization. Significantly, this failure to comply would affect the management of different patient aspects and could predispose them to complications. The non-compliance leads to poor prophylaxis assignment, leading to ineffective treatment and adverse events. The assessment using the Caprini tool is inadequate and, in some cases, missing. The lack of compliance and poor review are critical areas of concern.
